Terms and Conditions of Registration for Damira Basics and Essentials

Intro:

The following makes up the terms and conditions of your registration under our dental payment plan at Damira Dental Studio’s. It is recommended that you read them carefully and keep them in a safe place so that you can refer to them in the future, should you need to. Please remember, the agreement is with your dentist and cannot be transferred to another practice or dentist. The direct debit you have in place to cover the cost of your payment plan constitutes your agreement to the terms outlined herein.

Initial Term

The initial minimum term of your dental plan is 12 months. If you terminate your dental plan before this period, the practice reserves the right to recoup any appointment fees and any discount received on treatment will need to be repaid to the practice.

Your dental plan includes:

Basic’s Membership

  • 2 Dental examinations per year
  • Routine diagnostic X-rays usually taken two yearly
  • 15% discount off routine dental treatments
  • Global Dental A & E Assistance Scheme

Essential’s Membership

  • 2 Dental examinations per year
  • 2 Hygiene Visits per year
  • Routine diagnostic X-rays usually taken two yearly
  • 15% discount off routine dental treatments
  • Global Dental A & E Assistance Scheme
  • Complimentary private prescriptions
  • Complimentary emergency consultations

Monthly Fee and Direct Debit collection:

Your monthly plan fee will be collected by direct debit on the 1st of each month or shortly after.

Plan fee amendments:

Your plan fee is subject to change at any time. The practice will endeavour to review plan fees once per annum and should your plan fee change, you will be given advance notice by either post or email.

Plan Cancellation:

You may cancel your plan at any time outside of the initial term by informing the practice by telephone or e-mail. The practice reserves the right to recoup any appointment fees and discounts on treatment should you cancel during the minimum term of membership.

Non-payment:

If we are unable to collect your regular plan fee for any reason, we will contact you to collect any outstanding fees which can be collected by direct debit or paid to the practice directly by cash or card.

Refunds:

Dental plans are not usually refundable. A dental plan offers the ability to budget for your future dental care needs in a fixed monthly amount. This monthly amount covers you for future preventative appointments alongside additional benefits in practice including reduced treatment fees. Your plan payment also offers you access to The Global Dental Scheme for financial assistance in unfortunate or unforeseen circumstances. To ensure you seek maximum benefit from your dental plan it is important you book regular appointments with your dentist/hygienist as appropriate. In exceptional circumstances, that will be assessed and at the sole discretion of the practice manager, a refund may be agreed.

Having not attended appointments for a prolonged period will not result in dental plan payments being refunded as a dental plan has continued to offer other ongoing benefits to you that have been available should you have needed them. These include access to The Global Dental Scheme and registration with a dentist at the practice.

Failure to attend appointments:

You are responsible for keeping appointments made with your dentist and/or hygienist and you must pay any ‘missed appointment’ fee should you fail to do so.  You must ensure that you also attend your dentist for regular examinations, receive the treatment your dentist advises and you must promptly inform your dentist of any injury, problem or other material matter affecting your oral health; if you fail to ensure any of this you will be liable to pay any fee reasonably charged for treatment necessary to restore your oral health, which could otherwise have been avoided.

If you fail to attend an appointment covered under your plan you will be given two options:

  1. Sacrifice your plan appointment and remake an appointment once the required consecutive payments have been received.
  2. Pay a failure to attend fee and rebook your plan appointment immediately.

Complaints:

Please refer to the practice’s complaint procedure regarding any aspect of care or service which has not met your satisfaction.

Global Dental A & E Assistance Scheme:

Your dental plan includes access to a Global Dental Accident and Emergency Assistance Scheme.

The Scheme is established to offer support to patients who request assistance or treatment following an accident, a dental emergency, or who are diagnosed with mouth cancer.

Whilst the scheme aims to provide benefits in most cases, the scheme is a wholly discretionary scheme, not an insurance scheme. It has no obligation to provide benefits and the Scheme Manager will look at each request individually to decide whether or not to provide benefits.

Terms and Conditions of Registration for Damira Care

Overview

The following make up the terms and conditions of your registration under our dental payment plan at Damira Dental Studio’s. Please remember, the agreement is with your dentist and cannot be transferred to another practice or dentist. The direct debit you have in place to cover the cost of your payment plan constitutes your agreement to the terms outlined herein. It is your responsibility as a patient to ensure that you book your own dental appointments.

Plan Name: Damira Care Plans, all bands A-E

Initial Term

The initial minimum term of your dental plan is 12 months. If you terminate your dental plan before this period, the practice reserves the right to recoup any appointment fees and any discount received on treatment will need to be repaid to the practice.

Your Dental Plan

Your Dental Care Plan includes cover for your routine preventative appointments and for clinically necessary treatment, this includes:

  • Examinations (including required X-rays)
  • Scale & Polishes / Hygiene visits as clinically required and prescribed by your dentist.
  • Fillings
  • Root canal treatment
  • Routine extractions
  • Preventive dental advice and therapy
  • Crowns, dentures, and bridgework (excluding laboratory fees)

Clinically necessary treatment is treatment that is prescribed by your dentist following an examination that is required to maintain your dental health. It does not include elective or cosmetic procedures.

Your Dental Plan does NOT include:

  • Treatments not specified as included in the above list.
  • Referral to a specialist or specialist interest clinician which is necessary in the reasonable opinion of your dentist.
  • Any treatment which is considered cosmetic.
  • Any treatment which is not clinically necessary in your dentist’s opinion
  • Prescriptions
  • Laboratory Fees
  • Treatment undertaken at other practices during your registration.
  • Additional hygienist visits that are not clinically prescribed by your dentist.

If you require clinically necessary treatment that includes laboratory work (crowns, bridges, dentures) you will be required to make a financial contribution to this treatment that is equal to the laboratory fee incurred by the practice to produce the required work.

Plan fee amendments:

Your plan fee is subject to change at any time. Your plan fee will be reviewed on an annual basis. Should your plan fee change you will be given prior notice in writing either by e-mail or post.

Your dentist will, from time to time, review your current full care plan category to ensure we are able to allocate the correct amount of time with you to keep you dentally healthy. If your treatment/hygiene needs exceed the time your current category affords, your plan fee will be amended accordingly which will be discussed with you in person. This will affect your monthly direct debit payment and you will be given advanced notice of any change in monthly plan amounts.

Plan Cancellation:

You may cancel your plan at any time outside of the initial term by informing the practice by telephone or e-mail. The practice reserves the right to recoup any appointment fees and discounts on treatment should you cancel during the minimum term of membership.

If, after the second attempt we are still unable to collect the amounts due your dental plan will be cancelled and you will be moved to a pay-as-you-go patient in the practice.
