"booked in to see a dentist whom the member of staff on reception told me was told was exceptionally good with nervous patients. She was most professional, telling me at every stage what she was doing and continually checking I was comfortable. What to bring with you?
You will be requested by reception to update your medical history form on an annual basis as per the current GDC guidelines. It is useful if you have complex prescriptions, for you to bring these with you to your appointment so they can be copied to your records.

Any patient booking in as an existing patient who has not been seen within the last 36 months will be charged from our private fee list. If you are unsure if you qualify for NHS routine care please contact our reception team. If you are a new patient to the practice you will be charged on a Private basis. All patients who have not been seen at our practice before will be offered urgent care where capacity on the NHS allows us to offer this.
